Lead with Purpose in Dementia Care

What You'll Do

Leadership & Administration (60-70%) Lead a team of registered nurses ensuring exceptional care and full NYS DOH compliance. Oversee case assignments, develop individualized care plans, build deep relationships with clients and families, and spearhead quality improvement initiatives. Mentor junior nurses and lead clinical orientation for new team members.

Fieldwork (30-40%) Conduct in-home clinical assessments, manage complex cases requiring discretion and sophisticated clinical judgment, and maintain regular communication with families and healthcare stakeholders. Participate in 24/7 remote on-call rotation.

What We're Looking For

Must Have:

• Active New York State RN license

• Proven leadership experience managing nursing teams

• Genuine passion for dementia care (professional or personal connection to the dementia journey)

• Strong regulatory knowledge (NYS DOH experience preferred)

• Concierge-caliber interpersonal skills for high-profile families

Competitive Package $100,000 - $130,000 base salary plus team-based growth bonuses (every 1,000 hours of care) and annual performance bonuses. Full benefits including health, dental, and 401(k) matching.

Flexibility & Balance Hybrid schedule: 3 office days (Tues-Thurs), 2 flexible days for fieldwork/remote. Dedicated nursing office in accessible Midtown Manhattan.
